Abstract BACKGROUND The most widely used classifications of adverse events (AEs) in neurosurgery define their severity according to the therapy treat them. This concept has substantial shortcomings because it does not reflect AEs that are treated, such as new neurological deficits. OBJECTIVE To present a novel multidimensional and patient-centered classification AE evaluate its applicability. METHODS Therapy-Disability-Neurology (TDN) grading system classifies depending on associated therapy, disability, We conducted 2-center retrospective observational study 6071 interventions covering whole neurosurgical spectrum with data prospectively recorded between 2013 2019 at 2 institutions from countries. RESULTS Using first patient cohort (4680 interventions), positive correlation was found LOS well treatment cost. Each grade greater deterioration Karnofsky Performance Status Scale (KPS) discharge follow-up. When using same methods external validation (1391 correlations grades AE, LOS, KPS were even more pronounced. CONCLUSION Our results suggest TDN is consistent clinical economic repercussions thus reflects severity. It easily interpreted enables comparison different medical centers. standardized report scientific literature could constitute an important step forward toward critical, patient-centered, evidence-based decision-making neurosurgery.
